How to Fill Out the Parental Consent Form

Please follow these steps carefully:

• We provide consent forms in different languages.
• Important: You must complete the form in English.

2. Submit the Form:

• Fill in all details correctly before you submit the form.

3. Verify Your Email:

• After you submit the form, you will receive a verification email.
• Open the email and click the link to confirm your submission.
• Note: We cannot access or process your form until you verify your email.
